If your insurance is not yet accepted by Flourishing Tots, you can schedule here. Prepayment with a credit card is required for online booking. If you prefer to pay with cash (paper or venmo) or by check, please text me to schedule. HSA/FSA card are accepted.

Please add the appropriate out of office fee (home/virtual) when booking.

If you forget, I will need to decline your appointment and you’ll have to redo it which will result in delayed scheduling.

All appointments are pending until you receive notification that your appointment has been accepted.

If you have United, Aetna, PacificSource or Cigna insurance, head over to the insurance booking page.

 

If you are a current patient of Sunshine Family Healthcare:

  3. In the “appointment notes” section, please indicate the reason for your visit & that you are a patient of SFH so I can chart in the appropriate system.

  • You cannot book online less than 4 hours before an appointment time or more than 30 days in advance. Text me if you need an urgent appointment.
